The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with a clinician.
The practice can only accept patients who live within 10miles radius of the practice.
If you wish to join our medical practice, please contact the Receptionist via website or telephone. If you are already a Patient on the NHS and have a Medical Card you will need this with you. If you have mislaid your Medical card, you need to fill out a Registration Form know as HS200. If you wish to join our medical practice but have never been registered with the NHS before, you will need to fill out a Registration From know as HSC/R1. You will also need copy of ID (passport or birth cert accepted) and 2 forms of Proof of address. You are also entitled to register as an NHS patient, if you live in the ROI but work in NI.

Your rights and responsibilities are set out in our Patient Charter. Patients who are violent or abusive towards GPs or staff members may be removed from the Practice list. Patients who threaten or are violent towards any GP or staff member will be reported to the Police and further action pursued.
